In order to be considered for services and/or accommodations due to a disability, it is your responsibility to register with the disAbility Access Services Office. You must provide disAbility Access Services with documentation of your disability. |

To establish eligibility for services and/or accommodations.
To identify the impact(s) of the disability to determine appropriate services and/or accommodations. |

| Documentation Of Your Disability Should Include |
A diagnostic statement identifying the disability, including a DSM IV diagnosis, if you have a mental disorder;
A description of the diagnostic criteria and/or diagnostic tests used;
A description of the functional impact of the disability ( the severity and how it affects you);
Medications, treatments and services currently prescribed, including any medication side effects;
A description of the expected progression or stability of the impact of the disability over time;
Recommendations for accommodations and support services;
The name and credentials of the diagnosing professional(s).
disAbility Access Services staff are best able to provide equal opportunity when the documentation is thorough, complete and current (preferably no greater than three years old). Exceptions to this guideline will be made on a case by case basis. You will be informed of incomplete documentation and what is still needed.
Once your documentation has been received, you will meet with a disAbility Access Services’ counselor for an intake appointment to determine and make arrangements for services and/or accommodations. |
