Student Medical Form
|
1. The Student Medical form is part of your acceptance/alternate packet. Information is included in the packet regarding the DEADLINE date for returning this information.
2. The ENTIRE Student Medical form and the CPR documentation must be completed and turned in to the program department chair by the deadline date. Failure to do so results in losing your place on the acceptance/alternate list.
3. Please check the Student Medical form for completeness BEFORE leaving the Doctor's office. This will prevent a return trip.
4. CPR certification needs to be health care provider level.
5. A copy of the Student Medical form, immunization information, and CPR documentation should be made and kept in your personal files.
6. The department chair will review the Student Medical form and CPR documentation and notify you of the status of their completeness.
7. Students are responsible for keeping their immunizations and CPR updated. Those who do not are not allowed to attend the clinical portion of their nursing course and will be marked absent (see attendance policy) until the information is provided. |
