Records Office Forms

The Records Office offers several services that require a form submission. Forms are listed below.

There are three methods to submit non-electronic forms to our office. A photo identification is required with all form submissions that request updates to name, address, phone numbers, and/or email addresses. Please review each form for any additional documentation that may be requested.

  1. In person – You may submit Records’ forms to our office, located on the 2nd floor of the Medlin Campus Center at Jamestown.  We are open Monday-Thursday from 8 AM to 6 PM and Fridays from 9 AM to 5 PM.
  2. Fax – You may fax us the form with the appropriate documentation to (336) 454-7073.
  3. Scan and email – Please scan and email the form and appropriate documentation to records@gtcc.edu
